Diabetes creates stress — the cognitive load of tracking food, blood sugar, medications, and complications is genuinely exhausting. And elevated cortisol directly raises blood glucose — independent of what you eat.
Cortisol signals the liver to produce more glucose (gluconeogenesis) as part of the stress response — the body preparing for "fight or flight." In acute stress this is appropriate and temporary. In Type 2 diabetics managing a chronic, unresolvable condition, cortisol elevation is persistent. The result is a background level of hepatic glucose output that no dietary adjustment fully compensates for.
A meta-analysis of 24 studies found that elevated salivary cortisol was independently associated with A1C levels across all Type 2 diabetic populations studied — an effect that persisted after controlling for diet, exercise, and medication adherence.
The Loop That Standard Treatment Can't Break
Metformin and GLP-1 medications address glucose output and insulin response. They do not modulate the HPA (hypothalamic-pituitary-adrenal) axis — the system driving cortisol production. This means that for patients with high cortisol-driven glucose elevation, treatment is essentially managing the downstream result of an upstream problem that continues unaddressed. Many of these patients describe a sense that no matter what they do, the numbers keep creeping up — because they do.

What Addresses the Upstream Problem
